Incident reports for exposures and broken equipment.

When someone is exposed or a machine breaks, it is reported on the phone or the desk: what happened, when, who was exposed, the first aid given, and whether the manager was told. A manager closes the report, and it is kept.

Incident reports

What it does.

  • Exposure or equipment

Two kinds of report, each with what it needs.

  • Out of service at once

A broken machine can be taken out of service from the report.

  • Photos, with care

No patient in the frame, and no chart or day sheet.

  • Closed by a manager

Every change kept in the report’s history.
